04:53 PM EDT, 06/02/2025 (MT Newswires) -- Ensign Group ( ENSG ) said late Monday it has acquired the operations of Toluca Lake Transitional Care, a 52-bed skilled nursing facility in North Hollywood, California.

The acquisition, effective June 1, is part of a broader deal involving seven other facilities acquired from Providence Home and Community Care, first announced in December 2024.

Financial details weren't disclosed.

The real estate associated with Toluca Lake Transitional Care will be acquired by a subsidiary of Standard Bearer Healthcare REIT, Ensign's affiliated real estate company, pending state regulatory approvals, Ensign said.

Separately, the company also acquired the operations of Ironwood Rehabilitation and Care Center and Lakeside Rehabilitation and Care Center in Coeur d'Alene, Idaho, effective June 1.

The 80-bed and 100-bed skilled nursing facilities were added under a long-term triple net lease with a third-party landlord.
